- Physician referral forms if required by insurance
- A list of current prescriptions and/or over-the-counter medications you are taking, including dose and frequency
- Pertinent information about your medical and surgical history
- All appropriate medical records you have in possession
- You will be asked to fill out the following forms for your visit, which you can print out and complete before you arrive
